The format is the product.
This is the document your leadership, your GC, and eventually an examiner will read. If the format does not survive your most skeptical reader, no conversation is needed — that is the point of showing it.
Findings are written as state, evidence, and date — never blame. That wording is what makes the report safe to have on file, and it is the part most vendors will not show you in advance.
Commissioning a diagnostic sight-unseen is how vendors get chosen and regretted. Read the specimen first; then decide if a scoping call is worth fifteen minutes.

Cordera Regional Bank is a fictional institution. The fieldwork described in this document did not take place and every observation in it was written to illustrate the format of a completed assessment. Nothing here is a measurement of any real organization. Observations are written as conditions observed during the fieldwork window of 8-19 June 2026 and describe no other period.
Section 01
How to read this report
This document is a specimen. The institution does not exist, the fieldwork did not happen, and every observation in it was written to illustrate the format. Nothing here is a measurement of any real organization.
It is published ungated so that a reader deciding whether to commission an assessment can see the finished object first, in full, without a call and without us in the room.
What a real report changes. The institution is real and named on the engagement letter. The decision points are the ones actually mapped. The observations come from records and sessions that took place, on the dates recorded. The structure, the language, the states and the limitations page are the same.
The register this document is written in
The four states used in this report
- ON RECORD A named accountable role is recorded for this decision point, and the account was substantiated from records available in the room at the time of review.
- UNDER STRAIN A named accountable role is recorded. Parts of the account could not be substantiated from the evidence available at the time of review.
- UNTESTED A named accountable role is recorded and was not examined under questioning within this scope. Untested is not the same as sound, and it is not the same as exposed.
- EXPOSED No named accountable role was identified in the record for this decision point at the time of review.
Section 02

What was examined
